I acknowledge that I will maintain a center, institute, or laboratory that follows safe work practices and complies with all applicable policies and regulations, and that students will obtain any necessary approvals (e.g., IRB) and trainings.

I am a faculty member supervising an undergraduate student or students in a classroom or in an independent study or volunteer project.

My student or students are working on a project or projects that may require training or regulatory approval (such as work with human subjects),
and I acknowledge my responsibility to ensure they obtain any necessary approvals and complete any necessary trainings.

I affirm that the research in which my student or students are involved does not require any training or regulatory approval.
