*New as of 3/2/20* Supplemental SGA Approval Form (PDF attachment) – Once you have your supervisor’s signature, please forward this form to firstname.lastname@example.org for further approvals. Once all signatures have been obtained, students will receive the completed form that they must submit via the Approval Policy/Form link above. Students will receive communication from the CSSH Graduate Office when the submitted online form has been processed.
